Admission Procedures

Complete the following CERTIFICATE COURSE APPLICATION
in its entirety to apply for enrollment into one of
Century University's Certificate Courses.
Do not use this application if you are applying for one of our Degree Programs. Use the Degree Program Application instead.

All applicants must submit the following:

	The completed application or enrollment form.
	College/University transcripts. Student copies or photocopies if available. Official transcripts sent directly from the schools you attend will be required within 60 days of your enrollment.
	Documentation of the successful completion of any specialized training programs (degree related) that you are requesting credit for.
	Current resume - curriculum vitae or the University Information Form
	The quoted amount for the selected course in US $ using a credit card must be submitted before the application can be processed.
	Submit the initial payment in US$ by wire, money order, or personal check; also by credit card from USA or Canada only.
	The Application will be processed when the above requirements have been met.

Upon receipt by the university of your application, your application will:
(Foreign Applications will be handled via airmail, e-mail, or fax.)

Be reviewed by an evaluation counselor for a certificate course evaluation.
Following the evaluation, the counselor will call you to advise you of the results of your evaluation. You will be asked if you concur with the evaluation and if you wish to proceed with the certificate course and have your application for admission submitted to the Admissions Committee.

The Admissions Committee meets weekly. Your application will be submitted at the next committee meeting.

If your application is accepted by the university, you will be notified by telephone and also by a letter from the President of the university.
Your deposit will be fully refunded if you are not accepted.
Once accepted, if for some reason you decide not to pursue the certificate course, you must advise the university in writing within 5 working days of your written acceptance to receive a full refund of your deposit.
If after 5 working days you should decide not to pursue the certificate course, refer to the tuition refund policy stated in the university�s general catalog.

	Medical Coding: CPT Diagnosis Certificate Course $1,598		Medical Terminology Certificate Course $1,393

	Medical Coding: ICD-9-CM Diagnosis Certificate Course $1,393		Medical Transcription Certificate Course $1,598

	Medical Spanish Certificate Course $1,393		Pharmacy Technician Certificate Course $1,393

NOTE Certificate courses are designed to be completed in a minimum of five (5) monthly payments (starting second month of schooling) of $139.00 per month, at the beginning of each month.

Tuition for Medical Coding: CPT Diagnosis or Medical Transcription:

PLAN 1
Pay in full $1,598.00 at the time of application and receive a 10% discount.

PLAN 2
Pay $798.00 down payment at the time of application and five (5) monthly payments (starting second month of schooling) of $160.00 per month, at the beginning of each month.

Tuition for Medical Coding: ICD-9-CM Diagnosis, or Medical Spanish, or Medical Terminology, or Pharmacy Technician:

PLAN 1
Pay in full $1,393.00 at the time of application and receive a 10% discount.

PLAN 2
Pay $698.00 down payment at the time of application and five (5) monthly payments (starting second month of schooling) of $139.00 per month, at the beginning of each month.

No interest or finance charges for extended payment plans. All tuition must be paid prior to graduation.

I hereby consent to allow the university to list my name, city, degree, program, and employer�s name as �Directory Information� as defined by the Family Education and Privacy Rights Act, Title 20, United States Codes, 1232g, Subdivision (5) (A).

All questions I have asked have been answered by a representative of the university and I am satisfied with the answers given. I understand that the university is licensed with the State of New Mexico in compliance with Education Codes, 1978 21-23-1 to 21-23-15.

I certify that all information contained in this application is true, complete, and correct to the best of my knowledge and belief, and that all academic work I submit for my certificate course will be entirely the result of my own efforts, except for the use of text editors or translators if needed.
